Reimbursement

  1. Go to AIA’s website and download Claim Form Part I & II
  2. Submit the claim forms along with the supporting documents to AIA

What are the documents needed for AIA Medical Insurance Claim?

These are the documents needed for you to file your claim:

Hospital admission/ outpatient treatment

  • Individual Hospitalisation Claim Form Part I & II
  • Original detailed bills and payment receipts
  • Original inpatient bills (for hospital income benefit)
  • Copy of diagnostic report (if Part II is not completed)
  • Copy of histopathology report and original bills/ receipts for follow-up (for outpatient cancer treatment)

Third-party settlement

  • Copy of Settlement Advice issued by the third party
  • Copy of diagnostic report by the third party
  • Original medical bills/ receipts by the third party
  • Copy of medical test results by the third party (if any)

Overseas treatment

  • Copy of passport
  • Copy of flight ticket
  • Original detailed bills
  • Hospital discharge summary (if any)
  • Copy of medical test results
