This resulted in the formation of Takaful Malaysia on November 29, 1984, and the start of operations on July 22, 1985. After somewhat more than a decade of operations, the Company was transformed into a public limited company on October 19, 1995, followed by its listing on Bursa Malaysia Securities Berhad’s Main Board (commonly known as the ‘Main Market’) on July 30, 1996.

Original discharge note/summary providing the date of admission, date of discharge and diagnosis by the doctor
Original hospital bills/ invoices and itemised billing
Original medical receipts (including deposit receipt)
Duly certified test results/MRI/CT scan/dialysis card/biopsy/histopathology report, if applicable
Police Report duly certified (accidental cause)

Can I cancel my certificate?
Within the fifteen (15) calendar days of the free-look period, you may request to cancel your certificate. In this case, you may be eligible for a full return of your donations. If you opt to surrender your certificate after the fifteen (15) calendar days of the free-look period, you will be eligible for a part of your contribution refund, as decided by us, assuming you have not filed any claims for the current certificate year.
